Sales Tax Exemption Form

This form must be provided to the seller of the energy used in processing or agriculture to exempt future purchase from sales tax. Certificates are valid for up to 3 years in Iowa and up to 10 years in Wisconsin.

Please fill out all fields below to submit the form for your meter.

To submit the form for more than one meter, choose one of the following options:

• For less than five meters, please submit one form per meter.
• For five or more meters, please download the Multiple Meter Form and list all meter numbers to be exempt on a separate line. (Cities/Communities: For street lights with no meter, enter “LIGHTING” instead of the meter number.) Feel free to copy and paste from another source. Email the completed form to ProjectAdvisor@alliantenergy.com. Enter the following in the subject line: TAXES SUPPORTING DOCUMENTS – ONLINE FORM.

When to email supporting documentation:
• If you are submitting a Multiple Meter form.
• If you are claiming less than 100% (Iowa only). Use these forms: Electrical Exemption Percentage Calculations (Excel format) |Gas Exemption Percentage Calculations (Excel format)

IMPORTANT: When emailing supporting documentation, you still must fill out the form below one time. If you are sending a Multiple Meter form, use your first meter number listed on the Multiple Meter Form for the required *METER field.

Refund for Prior Period: To obtain a refund of sales tax previously paid on energy used for processing or agriculture follow these processes.. Iowa: Use form IA 843; worksheets and examples for completing an energy study can be found on the sales tax forms page of our website at https://tax.iowa.gov. An energy study must be attached to the IA 843 in order to receive a refund. Wisconsin: Use Publication 216 at https://www.revenue.wi.gov
