How to submit a claim form
When submitting claim forms, it is extremely important to fill out the paperwork completely, providing all necessary information, receipts, invoices or verifications.
Submitting an incomplete form will cause a significant delay in processing the claim which is not good customer service for either you or your customer.
If you repeatedly submit incomplete claims, your Energy Efficiency Dealer status may be revoked.
Each claim form must be complete:
If you submit both a heating and a cooling claim for the same customer, you need to make a copy of the receipt. Heating and cooling claims are processed separately; please do not attach everything together with only one receipt.
If you change the condenser and coils to make a certain SEER which is not in the ARI directory, you need to send corresponding factory verification to receive the correct incentive.
Mail completed forms and documentation to:

PLEASE NOTE: If you fax the claim form, please do not mail it.
Alliant Energy trade account managers will make decisions regarding the validity of claims. Program assistance and participation will be awarded on a competitive basis until all available funds are exhausted.
